Abstract: This report presented the first imported case of alveolar echinococcosis in Tianzhu Tibetan Autonomous County, Gansu Province. The patient was a local resident who had worked in Hotan Prefecture, Xinjiang Uygur Autonomous Region from 2019 to 2023, an area co⁃endemic for cystic and alveolar echinococcosis. He presented with hepatic discomfort in June 2023. Imaging examinations revealed an 8 cm × 6 cm mass in the left liver lobe, and serological testing showed positive IgG antibodies against Echinococcus. Post⁃surgical pathology confirmed the diagnosis of alveolar echinococcosis. Given that Tianzhu County has historically been endemic only for cystic echinococcosis and has not reported local cases of alveolar echinococcosis, this case was identified as an imported case based on epidemiological history. The case underwent left lateral hepatic lobectomy and no recurrence was seen at 2⁃year follow⁃up post⁃surgery. It is recommended to enhance the identification of different types of echinococcosis among clinicians, improve health education among migrant laborers, reinforce active screening among returning labors, and advance a One Health concept⁃based, cross⁃regional "human⁃animal⁃environment" joint prevention and control mechanism to prevent the risk of imported echinococcosis.

摘要： 本文报道了甘肃省天祝藏族自治县首例输入性多房棘球蚴病病例。该患者于2019—2023年在新疆维吾尔自治区和田地区（细粒棘球蚴病和多房棘球蚴病混合流行区）务工，2023年6月因肝区不适就诊，影像学检查提示肝左叶存在8 cm × 6 cm的占位，免疫学检查示血清抗棘球绦虫IgG抗体阳性，组织病理学检查示多房棘球蚴病。鉴于天祝藏族自治县既往仅流行细粒棘球蚴病，无多房棘球蚴病本地病例报告，故结合流行病学史将该患者判定为输入性病例。患者接受肝左外叶切除术，术后2年无复发。建议加强临床医生对不同类型棘球蚴病的识别能力，强化外出务工人员健康宣教和返乡后主动筛查，并以全健康理念推动跨区域“人⁃动物⁃环境”联防联控机制建设，防范输入性风险。
